Maybe 'cause the other ones knew him ou heard about him as a doctor (e.g. his former colleagues from BMRF and the rebels from C17). On the other hand, G-Man doesn’t only know him as a doctor, but also as a “submissive pawn”. But we can’t forget that he only mention “Mr. Freeman” at the end of the HL1 and at the beginning of HL2. In the HL2 ending and in HL2: Ep2, he calls Freeman as “Doctor”…
Gman is the only one who knows that Gordon is really the janitor posing as a scientist.
Mister and doctor are both accurate. You aren’t required to call doctors “Doctor” as a title. I believe that the G-man, however, purposefully used the term “Mister” to show that he wasn’t quite impressed with Freeman (yet).
That’s why the Anti-Mass Spectrometer broke, there was a penny in it!!

Actually in Episode 2 when the vorts are reviving alyx. And when G-man comes he says “dr. freeman”.
It doesn’t seem that important. You don’t always have to call doctors doctors.
Why arent my parents both called Doctor in this case?
Most people are just used to saying Mr. or Mrs./Ms. so they either forget or dont bother with special titles. Just normal stuff really.
