If you have to justify the whole Mr/Dr thing then there’s a simple explanation (though in likelihood it’s an error on Valves part)…

Other doctors and staff of Black Mesa are the seniors of Gordon (The events of HL1 occur soon after his employment) so refer to him as MR as they don’t see him as their scientific equal… and let’s be honest, Freeman never does anything more complicated than plugging in a machine, he’s a glorified lab assistant who just happens to be the right man in the wrong place.

As for G-man’s change of MR at the beginning of HL2 to DR at the end, perhaps the success of Gordon Freeman on his mission deserves a level of respect, so the subtle gesture of acknowledging your title may be the G-man’s means of giving a (shit) reward.
