I think Freeman’s Mind also mentioned aliens teleporting inside walls etc, which was one of the things making the place structurally unsound. That’d be interesting to see, too.
I rather want the teleports to throw away small objects because of a small shockwave. It wouldn’t damage to player, it’s just visual.

The way I see it, the random teleports sent aliens into low density areas, only occupied by gaseous matter. No telefragging or wall teleporting

On the subject of this telefragging, maybe it would be like on The Fly, the human and the alien combine to form a hybrid (Freemancrab perhaps).
In Kleiner’s lab in HL2, if I remember correctly, the mini-teleporter would swap two objects’ locations if they were on opposite platforms, so maybe the teleportations in Black Mesa did the same.
It would be kind of interesting if a scientist’s liver got swapped out for a houndeye. Of course, back wherever the houndeye came from, it would look as though it just turned into said scientist’s liver.
