The scariest monster to me depends entirely on my environment. Because, let’s face it, everything can be rendered harmless by being in the wrong place (…like a cage).

Alma. F.E.A.R. was one of the creepiest games I’ve ever played.
The only monsters that every genuinly scared me and made me reluctant of proceeding, were the Voltigores in the sewers in Opposing Force.
Anything else in any other game was just another moving target for me to shoot.
Stalker has a bunch of creepy monsters I must admit. Kudos to GSC.
Getting chased by the T-rex in Turok 2008 was pretty damn tense.
So was the part in the Antlion tunnels in HL2: Ep 2 where you’re getting chased by the Antlion Guard or Queen or whatever.
I agree very much, actually. Really because in Shadow of Chernobyl, all of the unmodded Bloodsucker encounters are indoors, as far as I remember, and they all scared the shit out of me, but once I modded it and they appeared outside, they were much less scary because you were extremely unlikely to get backed into a corner. Conversely with Call of Pripyat, I haven’t yet run into one inside, so they’re not nearly as scary. They are pretty freaky to look at, though.
And icthyosaurs scared the living shit out of me in HL, too. They were the one enemy that could get to you where no others could, not to mention that if you’re not playing HL:S, there’s no clear water where you could peer through to see if there was one in there, for all you knew you were jumping into the water with it staring at your back brandishing a steak knife and fork.
Any game with massive water beasts freak the crap out of me.
