The missing hivehand was patched/fixed several updates ago.
If it happened to you it could be due to a couple of things.
- Your saved game is corrupted. Restart the level from the new game menu.
- You are using mods.

It’s neither.
This is a result of a problem that (as far as I know) has been in the game since day 1. If you go back to the previous map, many of the entities in 5e will transition over with you into the skybox of 5d (You may have heard large amounts of explosions). When you go back to 5e, many of the entities will now be missing because they don’t transition back to 5e when the player does.
Just load a save that was before bm_c2a5d, or start a new game on the map itself (bm_c2a5e)

Yes, this might have happened; I’m not absolutely sure, but I believe this happened.
I had considered to restart at the chapter’s beginning, but I couldn’t be bothered to play it all again. In the end I just used giveitem weapon_hivehand
instead.
